Patients can manage their referrals digitally, integration between the NHS App and the Manage Your Referrals (MYR) patient website. It allows them to book, check, change and cancel their appointments online without help from the practice. See the link below

If you are pregnant you can self-refer to the maternity services by completing the self referral for antenatal care form. If you are going to self-refer, could you please contact the surgery to inform us and we will be able to update your medical records. Please send the completed form to Pregnancy.Referrals@bhrhospitals.nhs.uk

This is to ascertain whether you are a UK resident. You’ll be asked to provide ID at your first maternity appointment, usually your 12-week scan. If your appointment is soon and you won’t be able to take them with you, don’t worry, you can take them to your next appointment.

Referral can make by a Parent or the School Nurse directly by calling on 0300 300 188 option 1 option 3 to speak with someone regarding the self-referral. DO NOT need to ask GP for this referral.

This is an NHS service provided in partnership with our local GPs and other health care providers, to ensure a joined up approach to your care. Information is therefore shared with your GP to support continuity of care. Your information will be held securely and confidentially in electronic and paper format in accordance with the principles of the Data Protection Act 2018. All new referrals to our services will automatically be enrolled to use the Patient Portal, providing the patient has a valid mobile number and opted for SMS communication. The Patient Portal collects several clinical measures and questionnaires essential to your treatment and therapy.
